Extra business documents required by chambre de metiers?

Following my auto entrepreneur application and supplying some additional documentation, the chambre de metiers have sent a letter requesting the following:
- Nom de votre couverture maladie actuelle. On the application we answered couverture prive for maladie actuele, this was from the UK and has since expired, we named harmonie mutualite for the maladie choisi.
- A Carte Vitale - which we don’t have.
- Attestation de suivi du stage de preparation a installation.
Can you advise on how to answer these outstanding issues please?
