Applying for an A1 statement (to work outside of 黑料网最新入口)

I have recently moved to 黑料网最新入口 and created my business for freelance sound engineering here. I have a contract proposed for work in Holland, Belgium and Germany. The contract term will be from 13/09/21 - 28/04/22. I have been asked to provide an A1 statement to prove that I am paying social security contributions in my resident country (now 黑料网最新入口). I would like to know:

  1. Which form I require (and is the form the same for all business structures)
  2. If I need to return to 黑料网最新入口 during this contract to fulfill and obligations (such as residency being a holder of a 5-year Titre du Sejour) and,
  3. What I will need to complete this form
